COMLEX-USA Level 3 is a two-day computer-based examination assessing competence in the foundational competency domains required for generalist physicians to deliver safe and effective osteopathic medical care and promote health in unsupervised clinical settings.
Competency domains assessed include application of osteopathic medical knowledge, osteopathic patient care and osteopathic principles and practice, communication, professionalism and ethics. Competency assessment occurs in the context of clinical and patient presentations and systems-based practice as required for entry into the unsupervised practice of osteopathic medicine as an independently practicing osteopathic generalist physician and for readiness for lifelong learning and practice-based learning and improvement.
The examination contains questions in the multiple choice question MCQ format and 26 clinical decision-making CDM cases that include two to four questions each. Each exam question assesses one of 10 diverse, representative clinical presentations and one of seven competency domains for osteopathic medical practice.
While the MCQs have a single-best answer format, CDM questions are either extended multiple choice selection of one or more answer options from an extensive list that may contain more than one correct answer or constructed response short answer. The examination also includes audio-visual components designed to assess an expanded physician competency subset. Passing Level 3 indicates that the candidate has demonstrated competence in the foundational competency domains required for generalist physicians to deliver safe and effective osteopathic medical care of patients as required for entry into the unsupervised practice of osteopathic medicine and to continue lifelong learning and practice-based learning and improvement.

The first day of testing includes multiple-choice items divided into 6 blocks of items; 60 minutes are allotted for completion of each block of test items. There are approximately 7 hours in the test session on the first day, including 45 minutes of break time and a 5-minute optional tutorial. Note that the amount of time available for breaks may be increased by finishing a block of test items or the optional tutorial before the allotted time expires.
There are approximately 9 hours in the test session on the second day. This day of testing includes a 5-minute optional tutorial followed by multiple-choice items, divided into 6 blocks of 30 items; 45 minutes are allotted for completion of each block of test items. The second day also includes a 7-minute CCS tutorial. This is followed by 13 case simulations, each of which is allotted a maximum of 10 or 20 minutes of real time.
A minimum of 45 minutes is available for break time. There is an optional survey at the end of the second day, which can be completed if time allows. STEP Sixth Term Examination Paper Mathematics is a well-established mathematics examination designed to test candidates on questions that are similar in style to undergraduate mathematics. Other universities sometimes ask candidates to take STEP as part of their offer — in such cases, the university can advise on which papers to take.
